Evaluation Forms page
Roles, that can do this: Unit supervisor, Management and Admin -users (Team leaders and/or Coaches if given rights from Company settings)
The Evaluation Forms page provides a quick overview of all forms within the company. From this page, you can also easily activate or deactivate forms as needed.
Creation Date: Displays the form’s creation date and time. (A log of modifications can be found within the form itself.)
Lock Icon: If a lock icon appears next to a form, it means that responses have been submitted, so editing should be done with caution. If the icon is missing, the form has no responses.
Name of the form: Displays the name of the form.
Units: Shows which units the form is restricted to.
Roles: Indicates which roles can complete the form.
Status: Displays whether the form is Active (available for responses) or Deactivated (cannot be filled out).
Actions Button: Provides options for managing the form.
Edit – Opens the form for editing.
Show – Allows you to view the form structure. You can also export the form structure to Excel.
Copy – Creates a copy of the form and immediately opens it for editing.
Fill – Opens the form for completion. You must select the evaluatee from the dropdown list.
Passivate – Disables the form, preventing any further responses.
Delete Form – Admin users can permanently delete the form and all its responses from the system.
You can easily view a form’s structure by selecting Show from the Actions button.
The form structure can be exported to Excel by clicking Open in Excel from this view.

Managing Forms via the Actions Button

Viewing Form Structure
You can easily view a form’s structure by selecting Show from the Actions button.
