How to create/build evaluation forms?

Roles, that can do this: Unit supervisor, Management and Admin -users
If given rights Team supervisors (If given the rights), Coaches (If given the rights),

On the left navigation bar go to: Evaluation forms and + New evaluation form.

Fill the basic information for form:


First, give a name to the evaluation form.
Write a description of the form freely. You can also leave this empty.
Allow commenting of answers = users can comment on each evaluation questions when filling the form.
Allow attachments = users can attach PDF, JPEG and PNG files to the form. Maximum size is 30 mb/form.
Define units, where the form can be used. If it can be used in all the units, don't select any units.
Define roles, whose the form can be filled.
Add dimensions to the form. If there is none build, you can check the instructions from here: How to create dimension?
Select if the dimensions are Required when filling the form. This is recommended if you want to use dimensions in sorting reports.

Building the Sections and questions:

First, enter the title of the Section heading, for example, "Opening a contact", "Mapping", "Solution".
Under the section you can write the Question.
In the Description field below the question, you can enter additional information about the question.
Next select the Response options to the question. You can add more response options by clicking on the Add new answer button.
Decide, how much points each response will give. The score is converted into percentages on the form. One score is always a maximum of 100%. Responses and points can be for example Yes 10 points, No 0 points and Partly 2 points.
You can allow a whole Section to be by-passed by clicking the Allow skip sections. This allows users to bypass an entire section when making an evaluation.
You can also allow a single question to be bypassed by ticking the Allow skp box next to the question.
You can set a question as critical by ticking Critical. In this case, if the question in question is zero, the entire evaluation form will have zero points.
Add new Sections, Questions and Answers using the buttons provided.
When the form is complete, click Save at the end of the form. Please notice that the form will not be saved automatically!
Activate the form from the top of the form by turning on the Active switch. You can also activate the form from the Actions button on the Assessment Forms page.
