Dynamic Forms: Creation and Filling
Roles with Access: Team Leaders and Coaches, if granted the right to create forms, Unit Leaders, Management, and Admin users. Employees have access to fill out forms if allowed.

Once your company has acquired the Dynamic Forms add-on service, you will see the option The form is a dynamic form under Form Settings in the form creation tool.
Select Yes for this option to enable Multiple Choice Question and Branching Question options for each question in the form construction process.
With a Multiple Choice Question, you can create custom response options and set branching to desired sections or questions based on each selection.
When filling out the form, only the questions that are not included in Multiple Choice Question or Branching Question options will be visible.
For example, you can use a Multiple Choice Question at the beginning to determine what the respondent wants to fill out.
Please note that points cannot be assigned to Multiple Choice Questions, and in reporting, they will appear as empty fields.
Multiple Choice Questions are included in the Tracking Filter in reports, allowing you to quickly retrieve results for the desired forms.
If you have questions in a section that you want to appear only when a specific answer is given to a particular question, make that question a Branching Question and set the branching accordingly. In this case, remove those questions from the branching of Multiple Choice Questions. (More about this in the next section after the image).
A Branching Question differs from a standard question in that you can add branching from the question's responses to desired sections and/or questions.
For example, you can hide certain questions in a section behind a specific response option in a Branching Question. For instance, if a question in the section asks, "Did the customer service representative offer an add-on service?" you could set branching to additional questions for the "Yes" response to assess whether the offering was done correctly. For the "No" response, those additional questions would not appear.
Filling out a Dynamic Form differs from a standard form in that the respondent only sees the desired questions in specific situations.
For example, Multiple Choice Questions can be used to filter the form content to include only the desired sections.
Branching Questions can guide the respondent to fill out additional questions in specific situations based on their responses.
In the form, all sections and questions that are skipped due to branching will appear as bypassed in the report. These skipped questions are not included in the form's scoring.77
Note that the total score of the form includes the points hidden by Multiple Choice and Branching Questions during the filling process, but when saved, the points are recorded correctly.
Dynamic Forms appear in reports in the same way as standard forms.
The exception is that Multiple Choice Questions are included in the Tracking Target menu, allowing you to filter results. (Note: They are not included in the Tracking Target list in the Company Settings).
